If you are a new patient, you will need to register to create a "my chart" account. Then fill out our new patient forms after you select time and date for your exam with Dr Weiser.

Please Do Not schedule a cleaning if you have never been to our office, we would like to meet you first to to evaluate the work that needs to be done.

If you are an existing patient log into your my chart, or register if you don't have one yet.
